Teletha Testarossa (for Theresa Testarossa), 'Full Metal Panic'. (ADV) Luckily she is usually called 'Tessa', which is pronounced properly in the dub.
Alister Agrew (for Alistia Agrew), 'Last Exile' (Geneon). This one is particularly sad since Alistia's name is actually *pronounced* as 'Alister' in the dub, despite the fact that 'Alistia' is clearly audible as her name in the Japanese audio track. Since she is obviously a WOMAN, hearing her called 'Alister' in just the dub is just weird and confusing. Her name is spelled in Japanese characters here (on the bottom row, on the right) -

Lesson learned - be careful what you ask for, you might get it. Anime fans have complained for years about US distributors who tweak the names and spellings of characters or places to make them somehow more suitable for the US market, but in both cases above I believe it's the Japanese production company that came up with the ridiculous spellings, which were slavishly followed by the US distributor.
